1. Room type preference (1^st & 2^nd choice)
2. Room occupancy (total number of guests in the room)
3. Legal first and last name as shown on proof of citizenship for each guest
4. Country of citizenship for each guest
5. Date of birth for each guest
6. Primary guest’s contact information (address, phone number and email)
7. Credit card information (card type, number, expiration date, cardholder name and billing zip code)
