There are several parts to the application that must be filled out in order to process your membership. Please fill out the form in its entirety.
You will need to submit the following information:
1. King County 4X4 Search and Rescue Membership Application (this online form)
2. King County SAR Emergency Worker Application (here)
This form must be initialed on each page and signed before it can be accepted.
3. Proof of completion for ICS/NIMS 100b and 700a.

Download this document to your computer, then scan and upload the completed form as a PDF file below. Be sure to initial each page and sign the document. Please note that you do NOT need to include your SSN on this application.
This information will be used by King County Sheriff's Office (KCSO) to complete their database and perform background and driver's license checks, and by the King County Office of Emergency Management to register you as a volunteer emergency worker within King County.
Additionally, you will need to scan and upload a PDF of your proof of vehicle insurance, and a passport-style photo of you is also required.
First Aid / CPR
We highly encourage you to obtain a First Aid/CPR/AED certification before you respond on missions. The course must include a written exam and in-person skills assessment from an organization that provides verifiable credentials.
If you have already completed First Aid and/or CPR classes, please scan both sides of each card as a PDF file and submit them below. If obtaining a First Aid/CPR/AED certification will pose a financial hardship, please contact onboarding@kc4x4sar.org.
HAM License
A HAM license is beneficial but not mandatory. If you have already obtained one please upload your license below.

Additional information
King County will process your application for a background and driver’s license check. If you do not pass, you will be notified by phone of your ineligibility by the Sheriff’s office. You may re-apply when your records are clear.
You must also attend a General Meeting. We encourage you to take this time to meet with our members and ask any questions you may have.
After you have attended a meeting, your application will be presented to the Board of Directors, which will vote to accept or reject your application.
If the Board of Directors votes to accept your application, its recommendation will be presented to the general membership for a vote. Once the membership has voted you in, you will have one year to complete the required training.
