    Help us ensure that you and the staff of the AAHP can contact each other readily during an urgency or emergency while we are away from the studio. Please find our unlisted private cell phone numbers in the trip information we have mailed you. Do not broadcast our private lines, please. But be sure to have those numbers at hand!

    PLEASE NOTE THAT OUR CELL PHONE NUMBERS ARE NOT TO BE USED BEYOND THIS TRIP TO ATTEMPT TO DO REGULAR BUSINESS WITH THE AAHP.  SUCH CALLS RUN A VERY HIGH RISK OF NOT BEING ADDRESSED ON TIME; NOR CAN WE ADDRESS ANY ROUTINE BUSINESS MATTERS ON THOSE LINES.     By agreeing to participate in any activity/event organized, or attended, under the AAHP, you, the participant, agree to abide by all local, state and federal rules and regulations.

    By agreeing to participate in any activity/event organized, or attended by, the AAHP, you also agree to abide by all the formal rules and regulations posted by any and all vendors from whom the AAHP may be securing products and/or services, including event organizers, rented vehicle, hired drivers, food vendors, etc.

    You also hereby agree to the AAHP's rules of the road (ROR), and all other codes of conduct. These RORs vary by event and will be posted to you under separate cover when we invite you to use this form. You hereby also agree to the AAHP's photo and video policies as stated in the Employee Manual and/or client agreement.

    Please use this form to partake in the AAHP/Aii/S&B Studio outings, events, and other activities beyond regular classes. You may list multiple names on one form for the same event. Aii refers to ArtVentures International Ink., the Field School of the AAHP. S&B Studio refers to Skin and Bones Studio, the live figure and bone anatomy studio of the AAHP.  Aii/S&B Studio are not separate corporations; all Aii/S&B Studio matters fall under the AAHP.  All participants and their Representatives are subject to the Agreement of Policies, Terms and Conditions available on the AAHP website, or as an in-house copy for your reading convenience.

  • By signing the "Signature" box below this text, the Participant and/or his or her Representative hereby agrees to the following:

    To abide by the AAHP’s Agreement of Policies, Terms and Conditions, which can be reviewed on the AAHP website, or on our premises.

    To observe and obey all posted rules and regulations at any third party site or location we visit.

    To follow all instructions or procedures given by Aii or S&B Studio staff, or any other representatives of the AAHP.

    To acknowledge that fieldwork and field studio carry inherent risks, and to not hold the AAHP liable or responsible in any way for any injury or discomfort participant(s) may suffer.

    To not hold the AAHP/Aii/S&B Studio responsible for any lost, damaged or stolen personal property, whether at the AAHP or on any other premises.

    To not hold the AAHP and/or its paid employees, or volunteers, or any other AAHP representatives as responsible for any injury and/or sickness that is alleged as having been caused, or actually found to be caused, by your participation, and/or the participation of any member of your party, in this AAHP-sponsored event, its pre-event stages, or its aftermath.

    To assume full responsibility and not hold the AAHP and/or its paid employees, or volunteers, or any other AAHP representatives responsible or liable for any damages participant may cause to the facilities and locations visited through the AAHP/Aii/S&B Studio, whether through accidental actions, negligence, recklessness, and/or willful actions.
