I authorize all the schools, persons and organizations named in this application to provide any relevant information in their possession or knowledge to the agents of ServiceMaster, for use in deciding whether or not to offer me employment and specifically waive any required written notification. I hereby releaseServiceMaster, my former employers and all other persons from any and all claims, demands, or liabilities arising out of or in any way related to such inquiry or disclosure.

I understand that ServiceMaster is committed to maintaining a drug and alcohol free work place. I understand that if I am extended an offer of employment it may be conditioned upon my successfully passing a complete pre-employment physical examination. I consent to the release of any or all medical information as may be deemed necessary to judge my capability to do the work for which I am applying.
