Sign Out Board for Field Safety - User Information

Contact Information Instructions

This information will be used to locate you, someone who is likely to know where you are, and/or someone who will take the action to find you. The people who are on this list should be aware of this accountability system and accepting of the responsibility involved. The effectiveness of this list is based on reaching a contact by calling 5-6 phone numbers. The first person contacted will be expected to assume the responsibility of continuing to find you, so the list should be ordered with that in mind. A guideline of potential contacts is as follows:

1. You - your contact info should be first on the list
2. Your labmates - if you work in a lab with other RAs or PAs who would know where you are or how you could be found, however, some of these people may also be in the field with you
3. PI(s) - your PI (or PIs if working on a collaborative project) should be listed and should be aware of how to contact you and know how to get to the sites
4. Significant other / housemate - someone who you routinely spend your after work hours time with and would know where you are or how to contact the appropriate people have you found

Your contacts should provide the phone number(s) that they would most likely be at after working hours. Please let your contacts know that their phone numbers will be accessible when your tag is on the board. Also, please distribute the "Cary Institute Sign-Out Board Contact Awareness Letter" to all your contacts.

It is important to understand that the contact person is not necessarily responsible for physically going out and looking for you, but is responsible for taking the action to contact the appropriate group to find you based on your lab's Plan of Action.
