In the event of my employment, I agree to comply with all Blake Management policies, procedures and rules or other management communications as may be directed to employees. I also agree that if offered a position and hired, I may be required to successfully pass a post-offer/pre-employment physical assessment which may include, but is not limited to, any or all of the following (unless otherwise restricted by law): physical exam, drug-screening test, blood tests, TB test and urinalysis. Additional testing or work-related skills may be required prior to employment. I understand if I refuse to sign any employment forms of Blake Management, refuse to take any one of the tests, provide false information, interfere or fail to cooperate with the test, or test positive for any prohibited drugs, I will not be considered for employment. I understand that I will be given the opportunity to list any drugs that I have taken currently and/or recently before the actual test. I further understand that employment, if offered and accepted by me, is the result of a voluntary decision on my part to seek employment and a voluntary decision by the company to employ me. I recognize that my employment is not guaranteed or subject to any implied conditions or for any special duration, and that no verbal or written statements may bind Blake Management to any specific term or condition of employment. Blake Management is a senior housing management company that provides care and services 24 hours every day of the year. Employees are not guaranteed a specific shift. Shift assignment may vary depending on the resident census and needs.
I understand that Blake Management will complete a criminal record check and may also complete a driving record check. Any offer made will be contingent upon satisfactory results that are in compliance with state and federal regulations, as well as with the overall philosophy and values of Blake Management.
I understand that Blake Management reports any and all allegations of suspected or actual exploitation, abuse and neglect of the elderly to the appropriate state and local governing and investigative authority. I agree to participate fully in any state review or investigation. I understand that I may be placed on a leave of absence or suspension while an allegation is being investigated.
I also give Blake Management my permission for the use of any photograph or video taken of me while on duty during my term of employment. From time to time, such photos or videos may be used for Blake Management publications, including those used for internal communications and those intended to promote this company to the general community.
I hereby certify that the information given by me in this application and during the interview process is true and complete in all respects, and I agree that if the information is found to be untrue or misleading in any respect, I may be disqualified from consideration for employment. Untrue or misleading information on my application may lead to immediate termination if discovered after I am hired.
