In case you lose power prior to clicking the "Submit" button at the end of this form we suggest printing the form (or copying the longer answers to another file) as you progress through the questions. If you must stop mid-way, you can go ahead and submit what you have (after making a note of where you left off) and revisit the form later. The only fields required (for submission*) are site registration, Name, Email, and Marital Status, and apart from those there is no need to retype what you already submitted (though your answers may be saved in your browser for a day or so if you have autofill enabled). We will match up the answer sets based on your Name and Email.

SPECIAL NOTE ON NAMES: Please use the name that is on your CA driver license (CDL). If that is not the name you want to use on your documents, please get an updated CDL from DMV prior to signing your estate plan documents. The estate planning process usually takes several weeks, but then so does the DMV. US passport may also be used. However, most foreign passports cannot be used, and there may be issues with using DL's from other states. Permanent Residents note: GREEN CARDS CAN NO LONGER BE USED under California's restrictive notary law.
